Parkinson's Disease Early Detection Survey

We invite you to take part in a research survey being conducted by the
Movement Disorders Center at Glenbrook Hospital, Glenview, Illinois.  Dr.
Michael Rezak and his research associates are obtaining data through a
detailed questionnaire regarding potential early indicators in the
development of Parkinson's disease.

You must have an existing diagnosis of Parkinson's disease.  This survey
will be mailed to you and involve approximately 1-1½ hours of your time.

Early discovery of genetic and environmental factors causing Parkinson's
disease is critical for early diagnosis, effective intervention and the
arrest of Parkinson's symptoms.
