In Holland a disabled/elderly person living alone often gets a black box that hooks up to the phone with a dialer that calls in turn one of several caregivers/support organization. The dialer is triggered by what is essentially a garagedoor opener (transmitter). It is small and waterproof and worn as a necklace. It has helped my mother (who is 95 and still living alone) on a few occasions. I suspect they are available in the US.
