What contributes to quality of life in Parkinson's disease?

Quality of life in Parkinson's disease (PD) is most strongly influenced by
depression, according to this study, while motor impairment is not [!!!] a
significant factor.

92 patients with PD were assessed with the:
PDQ-39,
Mini Mental State Exam,
Schwab and England Scale,
UPDRS motor scale,
EQ-5D, and
the Beck Depression Inventory. (available per URL below)

Poorest scores on PDQ-39 were most strongly correlated with depression, as
determined by the Beck Depression Inventory.

Depression accounted for 54% of the quality of life score, while 10% was
due to disability as determined by the Schwab and England scale.

Additional contributing factors were postural instability and cognitive
impairment, while motor impairment as determined by UPDRS correlated less
strongly with quality of life.

The authors conclude, "QoL in idiopathic Parkinson's disease is primarily
determined by depression, disability, disease severity (particularly
related to axial features), and cognitive impairment.

"Patients with akinetic rigid subtype [!!!] have worse QoL than those with tremor
dominant disease [!!!].

"The motor part of the UPDRS, although the primary outcome measure for
clinical trials with antiparkinsonian medication, did not contribute
significantly to QoL in Parkinson's disease, and additional emphasis on
other disease features should therefore gain greater importance in the
treatment of disease."
