Dear Listmembers,

As some of you may know over the past several years, myself (a parkinsonian)
in
collaboration with the Human Interface Technology lab (part of the University
of Washington) have been developing special gait cueing glasses to assist
parkinsonians in overcoming walking problems. The purpose of the survey is to
determine how common certain experiences are among parkinsonians. By
responding to the following few questions you can assist us in furthering
this research.

AGE: _______

NUMBER OF YEARS SINCE YOU WERE DIAGNOSED WITH PD: ________

DO YOU HAVE PROBLEMS WITH TREMOR? _________

DO YOU HAVE ANY PROBLEMS WITH WALKING? _________

    FREEZING IN DOORWAYS? ________

    SHUFFLING GAIT? ________

    PROBLEMS STARTING TO WALK? _________

    PROBLEMS SUSTAINING WALKING? _________

DO EVER FEEL AS THOUGH YOUR FEET ARE "GLUED TO THE FLOOR"? __________

DO YOU FIND THAT STEPPING OVER OBJECTS ON THE FLOOR IS HELPFUL FOR WALKING?
_________

DO YOU FIND THAT YOU DON'T FREEZE WHILE WALKING UP OR DOWN STAIRS? _________

ARE SOME FLOOR PATTERNS EASIER FOR YOU TO WALK ACROSS (for example, cracks in
the sidewalk)? ___________

Thank you for your response. For your information I will publish the
results of this survey on the PD listserver.
