I don't know of a good book like you described... But...
Janet P and I obtained a Journal article that is very informative.

 I wish I had a scanner...and I would send it to you.
But I don't want to infringe on copyrights....so
you will have to go to your local library
and order one copy through inter-library loan.

Books are usually five years of date by publication time.
This article is 57 pages long, so it contains lots of information.
It is written for  the medical community...but it is
very readable. At the end, there is a long list
of references that you may also consider interesting.

As with any information, wisdom is required in the interpretation.
It will take you from diagnosis to end stage.
Every symptom is discussed, and the alternatives for treatment.
