See an experienced attorney who specializes in disability
insurance.  It made all the difference in the world to
me.  I was getting no place, when I had a similar
problem.  My union told me that it wasn't covered under
"the contract."

My attorney reviewed my case and medical records.  The
key information was in the underlying policy not the
information given to the employees.  My attorney
contacted the insurance company for a copy of the
underlying policy and then pointed out to them their
errors in responding to my claim.  I didn't have to go to
court.
