Factors impacting on quality of life in Parkinson's disease:
Results from an international survey
The Global Parkinson's Disease Survey Steering Committee
Movement Disorders 2002;17:60-67

Depression and disease severity are the major determinants of
health-related quality of life (HRQL) in Parkinson's disease, according to
this study.

Over 200 clinicians and 100 patients in six countries were randomly chosen
to participate.

Patients were assessed with the PDQ-39 (a PD-specific quality-of life
measure), Hoehn and Yahr scale, Beck Depression Inventory, Mini-Mental
State Exam, and other measures.

Multiple regression analysis revealed that disease severity (as measured
by Hoehn and Yahr stage) and dopamine use were significant predictors of
HRQL, together accounting for 17.3% of the variability in HRQL.

Depression had an "overwhelming" impact on quality of life (p<0.001), and
together with disease severity and medication, accounted for 58% of HRQL
variability.

Other factors with impact included "Satisfaction with the explanation of
the condition at diagnosis" and "Current feelings of optimism."

The authors note that a great discrepancy exists between patient-reported
depression (1%) and depression as determined on the BDI (50%).
