(April 2) with my Movement Disorder specialist group. It won't hurt to ask them if they would offer it to their other patients. HOWEVER, can't seem to locate a copy. I do recall having difficulty opening the attachment when you sent it last fall, but the problem was somehow overcome. I completed it and returned it, via email, to the stipulated recipient. I think I had to do it in two segments as I had not (and still have not) figured out how to send attachments. I know I can't open anything desinated "Winmail.dat" or something like that. Anything in MS Word works well though.
