A recent study which might be helpful is Clifford Davidson, ed., _Gesture in Medieval Drama and Art_ (Kalamazoo, MI: Medieval Institute Publications, 2001). The essays range from the theoretical to the pragmatic, but the volume as a whole probably summarizes what little has been done to date on medieval stage directions and might head you towards other more specific studies.
