I have no idea what to say except that my heart really aches for you!!  I don't know who to recommend you see or call but surely one of the doctors on our list can give you some advice on where to turn next.  They may not specifically know what is wrong but just knowing who to call next, or where to go next would help.  My only suggestion would be to go to Duke.  Tey have a great MK Udall Center and if you were admitted under the care of one of the Movement Disorder Specialists, they could deal with any PD problems and make sure you see the best specialists for everything else.  Let's face it, at Duke they consider unusual, difficult cases a personal challenge!!  Maybe you can get your neurologist to check into it for you.  It's worth a try!
As for the arrogant internist, I wish Robert would hit him once for every person on this list.  He desrves worse but that would do for starters!
