The duration of a dose of Sinemet varies widely from individual to individual, and also with the stage of the disease. IN GENERAL, regular Sinemet lasts from 2 to 4 hours, while extended release forms may last 4 to 6 hours. However, PWPs early in their disease may find that a single dose may be effective for considerably more time than that - allowing three times daily administration of even the regular form to suffice. As the disease progresses, it appears that the duration of action shortens. Sometimes increasing the size of a dose of Sinemet is enough to make it last longer. More often, a switch to the ER or CR formulation, or adding COMTAN or Deprenyl, adding an agonist, or switching to an agonist, are all strategies that help deal with the shorter duration of each dose. Keeping a diary with a schedule of "on-time" and "off time," as well as the times of dosing, is very helpful to the neurologist when trying to decide which strategy to recommend.
