Hi Joan:

Here is a little something developed amongst some list members
years ago... You still there backward walkin' George?

First... Put a three day supply of your current meds in a clearly
labelled EMERGENCY MEDS _ JOAN SNYDER zip-lok bag
and include explicit instructions and put it in your fridge freezer
door...

Second... Put a large fridge magnet "Emergency Meds Here"
on your freezer door...

Third... Put a card in your wallet or purse stating your condition
and current meds and where to look for more instructions...

Fourth... Get a "presentation type" three ring binder.  This is the
kind that has a see through spine and cover that you can slide your
own sheets in... Put a cover sheet in that says Emergency Medical
Information for Joan Blessington Snyder.  Print a similar identifier
for the spine.  I suggest using a fluorescent eye catching shade
readily available at Office Depot.  ( Other supplies you need are
paper, Tab dividers, vinyl sheet protecters, plastic business card
pouch sheets, and a photo album pocket sheet.)

Assemble the following information to suit yourself...

First sheet...

Your name, address, phone no. and your medical condition.

(Include any ALERTs such as Allergies)

Your GP's name, address, and phone no.

Your neuro's address and phone no.

Your current med schedule in detail...

Med.  -  dosage  -  time of day in military time  -  any pertinent
instruction such as with a light meal or ??

Second sheet...

Emergency contacts in order of who to contact first...

Example:

1.) Oldest son / daughter with name, address, phone, fax, cell,
pager, e-mail, work # etc.

2.) Closest sister / brother etc.

Third sheet...

Business cards for your GP, Nurse, Neuro, Pharmacist,
Employer, Union, Medical Plan, Optomitrist, etc.

Label each card My GP, My Nurse, MY Neuro, etc. and
use a fluorescent highliter to hi-lite each label...

You can include your neighbour either side; your hair dresser;
your lawyer, your best friend in Cincinatti...

You can include a business card for the kennel you want to look after
your dog... Your imagination reigns!!

Fourth sheet...

Photocopies of your prescription(s)
(include info re: where obtained...)

Fifth sheet...

Monographs of your meds...

* * *

Sixth sheet...

Photos of you, your spouse, your family,

* * *

Add the TAB dividers marked for easy reference...
Add or subtract anything you wish....

* * *

Keep this binder under the phone or some such high profile place
where it will be readily available if you ever need it....

Take it with you if you go on a vacation...

Keep a card in your wallet to tell folks to look for that binder
should you be stricken suddenly and unable to speak for yourself...
