The Pharmacia Patients In Need® (PIN) Foundation offers the Patients In
Need Program, which will provide immediate assistance to patients who
have limited income and are uninsured or underinsured (eg, no
prescription benefits).

Enrollment begins with the physician, patient, or patient advocate
calling the program at 1-800-242-7014, between 8 am and 8 pm EST, Monday
through Friday. Have the application with the attached certificate in
hand. Program specialists will collect specific information about the
patient, the drug therapy prescribed, and will ask for the certificate
number. If approved, a 30-day supply of the prescribed drug will be
authorized.

The patient completes section one of the application. The physician
completes section two of the application and returns to the program,
including the patient’s income documentation. Both the patient (or legal
guardian) and the physician are required to sign the application. The
application documents the patient’s eligibility for the program.
Approval of the patient’s enrollment will be based on current program
criteria and program eligibility secures an additional five (5) months
of assistance for the patient.

incorporated as a not-for-profit in New Mexico and headquartered in
Albuquerque. We are in the middle of applying for our 501C(3) federal
tax exemption status and expect to have it within 2 - 6 months. If this
exemption is not approved, at first, your donation may not be tax exempt
