Bill, et al: Jeannette has been hospitalized several times for various problems. I have a list of the medications in the form of a spread sheet. There are 2 sides to the sheet. The first side is the detailed timing of scheduled medications and the second the summary of each medication with the total daily dosage. Since she takes Selegiline HCL (Eldepryl), I specify that the Selegiline is contraindicated for use with Demoral which is a common medicine used in surgery. I take a current copy of the spread sheet data to each doctor visit and then the nurse asks if their charts are current with the medicines, I provide them with the copy. I am also able to fax a refill request to the pharmacy via use of another spread sheet form. Ain't computers wonderful? (When they work properly, of course.) It is very important to be able to provide this information to the medical staff in all cases as Bill and Winnie indicated.
