If you live outside the U.S. the rules may vary.  (For example, in Canada, I
believe the applicant's neurologist makes the assessment).  Inside the U.S.
(at least in Tennessee), there is a question on the application for renewal:
"Do you have a handicapping condition?"  Then it asks:  "Does this prevent
you from safely operating a vehicle?" (or something like that).  If you
answer "no" to the second part, there's usually no question.

Do other find this to be the case in their states?
