Phil,
I would guess that you are over medicated. There is a study  and slide presentation done by one of the MD researchers on Parkinsons showing that after about 5-7 years that Sinemet is effective only in a band of presence. So we are "off" to start the day and as we medicate we can go through the band and be "off" again. If we think that this means more medication and take more we will only be aggravating  the situation. I was going in that direction and suffering for it. Very agitated "off" times and not sleeping well.

After the addition of Amanadine and Trihexphenidy (generic) twice a day I have  cut back on the Sinemet from 4 doses of 50/200 CR to 1/2 of that. I take a quick release 25/100 and one 1/2 a dose of the CR to start the day. Then space out the other 1/2 doses of CR to fit my schedule. At bedtime I take another quick release 25/100 and a Benedryl pill about 30 minutes before actually sleeping. I usually sleep a full 7 hours. Some nights very well and others just well. 

As I work with my neurologist I get both of us smarter about my form of this disease. Since this is a disease that is different in each case what works for some will not be successful for others. Try cutting back on the Sinemet slowly about a half tablet a day until you have more "on" time than "off"time. At the point when you are under medicated the condition will mimic over medicated.  

When I am "on" I can drive normally, walk normally, type with both hands and do yard work or cook. In other words perform ADL's without assistance. I still have "off" times when I curse this disability and am not competent to button a shirt. Being willing to experiment some can lead to positive results.
