Experience shows that people will not put up with much of a
hassle before saying "the hell with it".  This is
particularly true if there is no direct benefit to the
person.  It's even more so if the task is physically or
psychologically stressful.

Therefore, if you want a good response, make it easy for
people.  For example, put the text-only version on the web
site; don't rely on someone on the listserv figuring it out
and spreading the word.

With somewhat more work and more expense, you can generate a
PDF form that people can fill out on the screen.  [Ray: the
file has to be created that way using the full Adobe Acrobat
7.0 Professional that costs $450.  If it's not, it does not
matter what version of the free Adobe Acrobat Reader you
have.]
