Sinemet is absorbed in the duodenum.   As you probably know, protein blocks
the metabolism of Sinemet.  It is possible that your wife has very slow
peristalses due to her PD and the food took a longer time to move through
her stomach and duodenum, thus preventing the absorption of Sinemet.
