Debbie, we are in California, and I don't know if it varies by state. 
When my husband stopped working about 8 years ago at age 53, he had to
go on state disability which required physicals, etc.  However, after
a few years, he was designated fully disabled and now receives Social
Security, no need for physicals or more forms. With something such as
PD, once he couldn't work any longer, I don't know why it didn't
automatically go to SS.  I wanted to scream at them asking if they
thought he was going to get better?
A little drama, but the forms and physicals were annnoying.
