David,
Thank you very much for posting this information. I have been treated now for 3 years for PD with Carb/leva and requip. They are working fine to control the trimmers. I have been having the same condition as you with my big toe and it never occurred to me to relate it to any medication. 
I certainly agree with you that we get precious little information from the doctors about what to expect from PD. 
Thank goodness for this and other web sights that tell it like it is.Wabler
