KF, good observation.  I don't know, but as compared to today,

Health care was pretty poor for everyone in colonial times as was the state 
of medicine.  In 1900  life expectancy was 45.

The enumerated powers of congress do not extend to education or any kind of 
"welfare".  A great deal has been done with  the "implied" powers of Article 
VIII #18, the "elastic clause" where congress can pass all laws necessary 
and proper to carry out the enumerated powers  like taxation, coining money, 
establishing inferior courts, regulating foreign and interstate commerce, 
declaring war, etc.

Health care would probably be justified under the interstate commerce power 
(although a stretch)., e.g my DBS NTs were made in a different state, my 
surgeon trained elsewhere, health insurance that is portable.

Opponents of "big" government want to go back to the days when a small 
government met the needs of a small nation.  They might even get rid of the 
postal service.  Or they could go back to the toll roads of the feudal 
system.  There is nothing in the constitution about interstate highways or 
aviation either.

The constitution is a brilliant piece of work as it can be modified to adapt 
to the times.
