Angie you should contact an SLP who specialized in PD.  LSVT as mentioned is helpful.  I believe they also do botox injections in vocal chords.  Finally, there are good amplifiers on the market today which might help you with voice strain.

I got a lot of help from LSVT voice training.  It seems especially designed
or PD.
For foot cramps, strenuous calf stretches help me a lot.
