I actually did that 2 years ago - got up on a broken shoulder.  The pain didn't hit for some minutes - odd, but my head was bleeding and I knew I had to get it stopped. (fell on one shoulder & head-butted a wall on the way down. The doctor said Ishould've been concussed but peered into my ears & said he could only see wax (what did he expect, a sea view ?)
