It is my understanding that iCare is preventative, not in home care exactly. 
my PCP recommended me.

THERE IS A SIMILAR PROGRAM CALLED "HOMEBOUND" USED BY MEDICARE  AND ONE
VERY SIMILAR ONE  ALSO CALLED "HOMEBOUND" THRU THE VETERAN  ADMINSTRATION.

A DOCTOR HEADS THE VA  TEAM BUT THEY ARE GIVEN  A REPORT , OF THE VISIT,
BY THE TEAM NURSE.  (  THE VISITING  TEAM CONSISTS OF A NURSE, CAREGIVER,
PHYCOLOGIST, PHYSICAL THERAPIST  AND SOMEONE TO EVALUATE WHAT IS NEEDED LIKE 
A
HOSPITAL BED/HOYER LIFT ET.  AND THERE IS ALSO A TELEHEALTH PROGRAM
AVAILABLE TO BE INSTALLED IN THE  HOME--IF NEEDED AT LEAST WITH THE VETERAN
ADMINSTRATION PROGRAM.

MEDICARE  HAS A VERY SIMILAR PROGRAM  (WITHOUT A PRIVATE SECONDARY
INSURANCE CO.) AND IT ALSO NEEDS YOUR PRIVATE  DOCTOR TO PRESCRIBE IT --BUT 
A
DOCTOR DOES NOT COME TO THE HOME--AGAIN THE TEAM  IS HEADED BY A NURSE AND 
SHE
"REPORTS" , TO THE DOCTOR.

AGAIN I BELIEVE THIS (WONDERFUL) PROGRAM---IT IS NOT  WIDELY PUBLICIZED.

I just  signed up with  iCare, a program offered by Banner Health that
replaces  my PCP with a team of 3 doctors, a  nurse and a Health Coach. 
This
program is for elderly people who have  hard time getting to Dr.  office.  I
get to keep my specialists like neurologist, etc.  I’ll  have
BP, etc. monitored electronically and by video.  The doctor who
interviewed me at home yesterday was perfect.  And he agreed with my 
reasons for
refusing  colonoscopy for IDA.

For someone who lives  alone this sounds good and costs me nothing due to
Medicare and Blue  Cross/Blue Shield insurance.  It will be interesting to
see how it  goes.  I’ll meet nurse and health  coach soon.
