I signed new consent slips last month to continue on the Cabergoline trial (location of the clinic I attend changed). The permission slip indicates drug company: Pharmacia Clinical trial stage 3 - long term usage. The consent slip signed by patient to participate in study should contain both drug company name and clinical trial stage. You are entitled to said information.
