The advantage of a system that is available in a retail store is the
opportunity to try it out before you buy it.
 
This little system that you can attach on the clothing has an on-off switch on
the microphone so it can be turned on when talking and off when you are not
anticipating talking to anyone. The volume control on the speaker its' self
will compensate for the feedback you describe. As with any voice amplification
system, limitations decrease as the operator becomes experienced in its
operation. The doctor also told me some patients carry the speaker in their
pocket.
