You are right.  I would assume the same limits that apply to the other
surgerys would also be applicatble to stimulation surgery.  I would assume
that the stimulator could be removed and the patient claim to be without
surgical intervention.  The wording on most drug progrms I've seen states
disqualification if a surgery has been performed.  Since so much
documentation must be submitted on each participant, the mention of a PD
surgery even of this type would most likely cause the participant to be
rejected.
