David - Thanks for your efforts to get Parkinson's included in the survey.
 In answer to your question about sense of smell:  Dr. William Koller of the
University of Kansas Medical Center has been involved in or participating in
the study of the sense of smell.  My husband took the test a number of years
ago as part of this study, and like most of the respondents on this list,
discovered that he couldn't distinguish between two very disparate smells.
 (I was in the room when he took the test, and although I was seated  about 3
feet away, I could clearly identify the scents - for example, when I clearly
smelled bubble gum, he was trying to decide whether he was smelling violets
or fuel oil).
If you would like more information, I would be happy to call KU Med and see
