     Recognizing Nonmotor "Off" Symptoms In Parkinson's Patients: Way
     To Avoid Unnecessary Treatments

     WESTPORT, Dec 11 (Reuters) - Recognition of nonmotor "off"
     phenomena in Parkinson's disease patients can prevent unnecessary
     tests and treatments, two researchers at the University of
     Medicine and Dentistry of New Jersey, New Brunswick, report.

     Drs. Jacob I. Sage and Machteld E. Hillen studied 130 patients
     with Parkinson's disease and motor fluctuations.

     Dr. Sage told Reuters Health: "Most physicians who think of
     fluctuations in Parkinson's disease patients think of motor
     fluctuations. An important thing to realize is that there are
     nonmotor fluctuations that may be just as disabling. This is not
     an uncommon problem. Among patients who have motor fluctuations,
     17% have nonmotor fluctuations, as well, [such as]
     cognitive/psychiatric dysfunction, sensory symptoms and autonomic
     dysfunction, which can occur as end-of-dose phenomena."

     More specifically, nonmotor "off" states, according to Drs. Sage
     and Hillen, are manifested as "...sensory dyspnea, nausea, facial
     flushing, cough, hunger, unilateral limb edema, proximal limb
     pain, and trigeminal neuralgia-like pain."

     The authors recommend diminishing the amount of "off" time as
     much as possible. To that end, "...[a]dding a dopamine agonist,
     using controlled-release preparations of [levodopa], or
     increasing the frequency of dopaminergic medication may be
     helpful [in] some of these patients."
