Lynn,
I found the forms from S.S. to be pretty self explanatory. I was
able to requesty them via a phone call. Then read through them and called
SS office to get clarification on anything that I didn't quite understand.
Your primary care physician and your neuro specialist can be VERY helpful by
writing testamentary letters of your disability to include with your
application. The required interview can be done in your home so don't let
them call you into their office where you may have to wait for several hours
to be interviewed. All of the paperwork for the SSDI for my wife was
handled over the phone and in home interviews. Don't understate your
disability on the forms. A little exaggeration is OK but not much. Time is
seemingly unimportant to these people so don't hesitate to shake their cage
every two to three weeks to keep them alert! If more than a couple of
months go by without success, don't be bashful about calling your U.S.
representative's office and requesting assistance from one of his/her qaides.
