I posted this phone number awhile back, but maybe it's worth re-posting.

The Pharmaceutical Manufacturers Association has a hotline:

1-800-762-4636.

They'll mail you a booklet to take to your physician. If your
prescription is listed in the booklet, your doctor can request that you
receive it free. You may need to fill out an application, but it's worth
it.
