Naomi, Ken, and others:  Ken is correct. One of the aims of having a
Pharmacist review the charts of all nursing home patients at least once
each month is to try and reduce medications to the least effective dose.
this can be accomplished better in the NH setting because the nurses and
Aids can moniter the patient better than at home. Doctors tend to
believe the nurses reports more than they do the family. (This is not as
