These are some bloopers that some medical transcriptionists have received
from doctors...enjoy!


Mouth exam showed a missing canine.

We discussed having her breasts and Pap smear all in one place.

She is wearing thick glasses and a hearing aid in her right ear.

He denies seeing the room spinning or vomiting.

She has severe tardive dyskinesia of the hands, which are running away.

This elderly male is brought in by his caretaker because of the way his face
looks.

Another vehicle was changing lanes and struck the patient's rear end.

Her husband is leaving on a trip, and she can't be without her leg very much
longer.

The dizziness is caused by quick changes of the head.

We are going to change his Foley catheter once every six years.

Her knee seems to be bothering her more, as has her husband.

He is trying to do more activities such as golf and getting back into his
aquarium. (Hope he has a 100
gallon size!)

Gave her three urine samples to return.

The patient is chronically depressed from living alone and having a dog. (Oh,
sure, blame Rover!)

Patient was given lidocaine with epinephrine and easily removed.

Appetite seems to have improved, since in the office the baby was eagerly
eating his bottle.
