I agree with the advice to see a movement disorder specialist. It may
even be inapropriate for him to be taking any sinemet at all! The first
question to be answered is, Does the sinemet do him any good? It sounds
to me like he is a possible candidate for a diagnosis of one of the
parkinson+ syndromes. There are three signs that a person has an
atypical parkinsonism.:

        1. does not respond to sinemet

        2. does not have a tremer

        3. the most prominent symptom is a walking and balance
problem            from the very first.

If this is the case one should suspect atyical parkinsonism. The bad
news
is that none knows the cause or anything to do for it. The good news is
that you won't be spending money on worthless medication and you won't
have any of the side effects.

I have looked into this case because I have been diagnosed as a case of
atypical parkinsonism. For the last 6 months have been unable to walk. I
am totally wheelchair bound.
