Dr. Iacono's website provides a great explanation for pallidotomy vs.
stimulation and when each is appropriate.  You can find the discussion at:

would, however, like to caution you that Dr. Iacono's pallidotomy success rates
are typically much higher, causing him to choose pallidotomy over stimulation
more often.  There is no doubt that a well-placed pallidotomy lesion provides a
much better level of success than a stimulator.  The stimulator operation does,
however, require less of a learning curve and tends to be less risky in the
wrong hands, causing most surgeons to opt for the stimulator.
