the study you mention, written by M.Stacy and J.Jankovic in 1995,
is available at simon coles' 'James' archive site:
assuming you have web access, of course:

1. go to his site:
        Searchable archive of the PARKINSN list
        http://james.parkinsons.org.uk/search.htm

2. click on the 'hot link' to the search page
and then enter the following phrase in the search query window:
        PD & PD+ Syndromes

3. you should then be presented with a list of postings
including all four parts of the report as originally posted
by john cottingham

click on one of them to look at it
you could then print it, download it, or mail to yourself, or whatever


for whatever it may be worth, dr. jankovic is listed as one of the authors
in an abstract which i posted a couple of days ago:
