I apologize for the way the survey report looks; I tried to get the info in column using tabs and space bar but it didn't work well. Also, I started out saying PL=physician's letter, then proceeded to refer to it as DL (doctor's letter) throughout. That's what I get for rushing to finish (was expecting 22 for dinner last night).
