Dianne, Tolcapone increases the duration of action and thereby the ultimate blood level and effects of levodopa. Therefore it also increases the side effects- one of which is in some people hallucinations, If the hallucinations are bad call you mum's doctor immediately. He probably will lower the sinemet dose. With the hallucinations it is important to be sure she is safe. For instance not likely to act on a threatening hallucination and harm herself or someone else inadvertently. Her doctor should assess that at least by talking with you. In an urgent situation you should feel free to call your doctor or his backup at any time of the day or night. Usually if you call a doctor's office they will have an answering service or another number you should call in an after hours emergency. One should use this only in a significant emergency but use it if in your opinion it can't wait until morning. Physicians like anyone else don't like to be awakened but most would rather get a 3 am call from a patient or family member than from the police or the coroner because something that could have been done wasn't.
