Dr. William Koller, a well-known PD specialist and chair of the KUMC neurology dept. addressed this same concern in a recent newsletter, to wit: "This is an important question that has not been answered. Depending on the patient and the particular agonists being used, there are several ways to change from one agonist to another. Options include: 1) titrate the old drug down and then titrate the new drug up. 2) titrate the old drug down while titrating the new drug up simultaneously. 3) switch from one drug to another immediately, if the dose equivalent is known." You probably wanted one right answer, Patsy, but evidentally it's "different strokes for different folks." I can tell you my own experience in changing agonists several months ago, which was from Parlodel to Mirapex. My ND (who works w/ Koller) used option one above but went down very rapidly. I think it was three days to go off the Parlodel before I started the Mirapex. Tasmar is not an agonist. I just started it last week, and my ND said no titration was necessary. He prescribed 100 mg 3 xs a day. But, like everything else when we are talking about our meds, it seems we all react somewhat differently, so when it comes right down to it, you are the final judge of what is working and not working for you.
