I encourage you to consider B12 deficiency. This comes on very slowly, as the
body recycles B12 quite efficiently. The slowness of onset of deficiency
resembles Alzheimer's and has led to misdiagnoses. This is particularly important
now, as many companies are fortifying foods with folate (due to the connection
between folate and spina bifida in infants) and folate adequacy can mask B12
deficiency.

If your neuro has not considered this already, I would bring it up.
