I suggest you get your brother-in-law to the nearest university medical
school that has a movement disorders clinic or a Movement
Disorders/Parkinson research group and make an appointment with the first
available professor of neurology to properly diagnose him.
There is Parkinson's disease and there are Parkinson plus disorders which
are all very difficult to diagnose.  Internists or general neurologists
cannot do a proper diagnose because they do not see sufficient cases to
be familiar with these disorders (including Parkinson's disease.
