Tim, your vocal cords have become thinner, and therefore do not allow the air going through to be moduated properly. There is a procedure which involves injections of bovine collagen into the area, which takes a few minutes, and is highly effective, and after it is done a few times, lasts indefinitley. I am not a doctor, nor am I a victim of PD, but I learned this by reading this list. For details, you should ask Barb Mallut, who has had the procedure, twice, I think. There is also a therapy called Lee Silverman Therapy, which involves training of the voice, and I have also heard it works.
