In UK we are required by law to report our PD to the British Driving
License Agency and to the car insurance company.

FYI here is the PD questionnaire to be completed if you report you have PD
to the  British Driving License Agency

Medical Fitness to drive
We need the following information to enquire about your fitness to hold a
driving license. (Name address, DOB and doctor details)

YOU ARE LIABLE TO PROSECUTION IF YOU KNOWINGLY GIVE FALSE INFORMATION.
PLEASE ANSWER ALL THE QUESTIONS. IF YOU WISH TO ADD ANYTHING EXTRA PLEASE
WRITE ON A SEPARATE SHEET. IN CASE OF DIFFICULTY ANSWERING ANY QUESTION,
YOU MAY WISH TO CONSULT YOUR GP.

ABOUT YOUR MEDICAL CONDITION

1.  Do you have Parkinson's Disease? NO/YES
1a. Please give date of diagnosis.
2. Are you taking medication? NO/YES  If YES, give details and dosage.
3. As a result of your medical condition, have you or do you intend to
drive a vehicle fitted with special controls or automatic transmission
which enables you to overcome the effect of the condition? NO/YES
4. Do you ever suffer from freezing or the inability to move? NO/YES
4a. If YES, please describe these attacks
4b. Have you had such an attack within the last 3 months? NO/YES
5.  Do you have serious problems with memory? NO/YES
6.  Do you have episodes of confusion? NO/YES
7.  Have you misused alcohol or suffered from alcohol-related disease in
the last 3 years? NO/YES
8. Have you misused illegal drugs in the last 3 years? NO/YES   If YES,
please give dates and details.
9. Have you had an epileptic event(s) of any kind? NO/YES       If YES,
please give dates of first and last attacks and whether awake or asleep.
10a. Have you had any sudden attack(s) of disabling giddiness within the
last 3 Months? NO/YES
10b. Have you had any sudden attack(s) of disabling fainting within the
last 3 months? NO/YES
10c. Have you had any unexplained blackout or loss of consciousness within
the last 12 months? NO/YES
If YES, to 10a, 10b or 10c please give dates and details.
11. Can you read a number plate at 20.5 metres (67 feet) in good  daylight
(with or without the aid of glasses or contact lenses)? NO/YES

They also want permission to contact your GP and neuro for details.

From their letter:

It may take us some time to complete our enquiries. You are therefore urged
to seek specific advice from your doctors or specialists about whether or
not you should drive in the meantime.

When the enquiries are complete a decision will be taken about your licence
and you will of course be informed. The possibilities are:

     your license could continue as now

     your licence could be withdrawn altogether

     your licence could be withdrawn, but replaced on application by a new
one. This might run for one, two or three years

     your licence could indicate that special controls need to be fitted to
the vehicles you drive
