Mail went before I was ready. Don't know what happened. A fellow PD caregiver gave me the name of this workbook which you can get from Physicians' Disability Services, P. O. Box 827, Arnold, MD 21012 (410)431-5279 or from the Lupus Foundation. If you get it through the publisher in MD, you will pay $19.95+$4.50 S&H. Through the Lupus Foundation, you will pay $26.00 total. It takes you step by step through the application process for SSDI and includes many good examples, even examples for your doctor on how the letters must be written, etc. While we have not filed for this, it looks like something we can use.
