If Jim can't use anything but Sinemet, and has poor "on" time, have you tried
protein adjustment? If not, you might consider a two-week trial of Hearty Balance
products. You can order from their website:
http://www.balancenutrition.com/

If he gains more "on" time, then you may wish to consider using a
high-carbohydrate daily for him. (I'm not connected with HB in any way, though I
have met with two of their people.)

If you don't wish to order the Hearty Balance, you can make a 7:1 shake with
Ensure (NOT Ensure Plus!):

8 oz Ensure
1 cup sliced strawberries, peaches or other fruit (fresh, frozen, or canned)
2 TB sugar or honey
Place all in blender, blend till smooth. Makes 1 serving.

Also, some people have found that use of "liquid Sinemet" improves their on times.
The list archives have the recipe for this.

My very best wishes to you both,
Kathrynne
