I'm no expert, but a couple of ideas come to mind: County resources for the aged in the telephone book. Also your hospital social worker. Conservatorship? Elevator-stairs? Taxi-cab? (I have a friend with many elderly relatives and she has in some instances found taxis to be a better solution for them than owning their own cars s at a certain point).
