In general, drugs must pass TWO obstacles to reach the brain. You
mentioned the blood-brain barrier, but there is also the problem of
getting the drug into the blood. Drugs taken orally reach the blood
through specialized cells in the walls of the gastrointestinal system
(including mouth lining and tongue). When that's undesirable (for
example, apomorphine p.o. makes you vomit) they try the injection
or nasal lining route. In a trial of apomorpohine, some of the nasal
subjects had to drop out because their lining got too irritated.
