Elizabeth, It is just like AIDS. Nobody dies from it because it is not the aids virus that kills you. What kills you is one of several diseases you are normally immune to but which now attack you unopposed because AIDS has destroyed your defense mechanism. "Complications of ..." means that the person did not die from the whatever, but that the whatever caused conditions to exist (which otherwise would not have been present) which allowed another agent to inflict mortal damage. For example, if PD prevents a person from getting a normal amount of exercise and this lack of exercise causes them to expire three years ahead of schedule then PD was a complicating factor in the death.
