I experience an end of dose cramping in my right foot (my PD side)
that generally occurs first thing in the morning before my Sinemet has
had an opportunity to kick in.   I thought it might be related to
taking my morning Sinemet at first since this is generally when I
experience the cramping.  However if I have my shower before taking my
meds I still experience the cramping.  If I forget my meds or am under
a lot of stress this can and does also occur at other times of the day
but is most apt to happen at the end of my meds.

The cramping is a turning inward of my foot and a curling under of my
toes (like clenching your fist, only an involuntary and often very
painful toe clenching).

I deal with this cramp by stretching my achilles tendon and by
stretching the underside of my foot against a wall or a curb.  When
you are out walking and this cramp occurs you can get some instant
relief by walking backwards.  This is a trick I learned from George
Lussier here on this list.  Stretching on a daily basis is the best
prevention.  A good physiotherapist can help a lot here once they
understand the problem.
