Thanks for your input.  I took a look at my prescription and found the
company to be Pharmacia Upjohn.  A quick Yahoo web search for
"Upjohn and Mirapex" produced a number of sites with info.

This was found:

          Patients in the U.S. who have questions
           about Mirapex can call Pharmacia & Upjohn's
           patient product information number
           at 1-800-432-4702.

I just got off the phone with a very knowledgeable and friendly
person at Upjohn, who is sending me the titration information.  She also
told me the schedule (which will take a couple weeks to arrive
at the maintenance dose recommended by my neurologist). She
asked several times if I had this info from the pharmacy.  All I got
from them was the side effect sheet and nothing is mentioned about this
schedule.  I will forward it to my neurologist for his information too.
